Guts:Layout 1 6/12/08 7:49 AM Page 19 Routine Laboratory Tests ✓ Erythrocytosis and mild leukocytosis are sometimes present. ✓ Hyperglycemia is present in almost all cats and many dogs. ✓ Glycosuria due to hyperglycemia associated with diabetes mellitus. ✓ Proteinuria of renal origin is common in cats secondary to glomerular injury possibly because of the diabetes mellitus and hypersomatotropism. ✓ Azotemia and renal failure are common in advanced cases of acromegaly in cats.

Renal ✓ Polyuria and polydipsia are present in about 50% of cases and are caused by either increased renal blood flow or concurrent renal failure. Cardiovascular ✓ Heart murmur or gallop rhythm is ausculted in about 50% of cases. ✓ Congestive heart failure can occur secondary to hyperthyroidism, but is uncommon.

Hyperadrenocorticism must be ruled out in these cases. ✓ Finding a mass in the pituitary gland on CT or MRI is suggestive of acromegaly if the clinical signs are appropriate and hyperadrenocorticism is not present. Treatment Recommendations ♥ If acromegaly in a dog is due to progestogen excess, cessation of progestin administration or ovariohysterectomy should be curative. ✓ If present, diabetes mellitus may resolve if the acromegaly has not been long-standing. ♥ It is difficult to reduce the excess growth hormone secretion in cases of pituitary tumors, so much of the treatment of acromegaly is directed managing the complications of the disease, including diabetes mellitus, renal failure, cardiac disease and failure, and arthropathy.
