While complications of pregnancy and childbirth have been associated with anxiety, neither the duration and pattern of variation in the three trimesters, nor the behavioral correlates of anxiety, were considered. Increasingly, pregnancy is recognized as a critical developmental stage, one that challenges the adaptive and integrative capacity of the expectant woman and her family. A developmental conceptualization of pregnancy implies a continuum of reproductive health and adaptation, in which prenatal adaptation may influence the childbirth process, subsequent maternal adjustment, and maternal-infant relationships in the postbirth period.

It is essential that each investigator define family within the context of the research. Nurse researchers may substantiate a category of relationships that meet the roles and functions of family but do not meet the biological or legal definition of family. These relationships may constitute a person's "chosen" family, which nurse researchers may identify as an important resource in nursing interventions (Howard, 1976). Measures of family health or well-being have not been a central concept in nursing research (O'Brien, 1979).
