Research into non-impaired children’s pain threshold indicates that parental assessment is generally more accurate than those of nurses (Craig, Lilley and Gilbert 1996). There is increasing evidence that parents are best qualified to assess behavioural changes that indicate their child is unwell (Callery 1997; McGrath et al. 1998). This is largely because parental caring skills have been learnt over the years and parents often describe knowing their own child ‘upside down, inside out and back to front’.

A concern expressed by parents is the effect that pain and discomfort can have on their child, not just in terms of suffering, but also in terms of physical assault caused to their child’s already vulnerable state.

This has led to duplication of information and causes frustration to the patient and their family. Additionally, models are built around a process or expected progression, for example child development assessments comparing individual progress against the expected norms or Pathways of Care, that score deviation from an expected recovery, such as following surgery.
